Enrollment is Easy!

Enrollment for the 2017-2018 school year now open!

Enrolling your child in Kids Club 360 before and after school programs is easy! You can complete much of the enrollment process online. However, we are required to keep some documents on file - we'll need paper copies of those items. Just follow the steps below to register.

All enrollment steps must be completed & all paper documents received 24-48 business hours prior to your child's first day of care. Care will NOT be provided until enrollment has been completed.  

 

1. Gather the Paper Documents You'll Need

Universal Signature Form (Click Here)
Your Child's Current Photo
Your Child's Birth Certificate 
Your Child's Immunization Records 

Medication Administration Form (if applicable-Click Here)-MUST be signed by a doctor. Medications, EpiPens, and/or Inhalers must be turned into the Central Office or Site Director prior to youth starting the program. 

Action Plan Packet (if applicable)-Must get from your doctor for asthma, allergies, and/or seizures. 

Guardianship Documentation (if applicable)

Discount Documentation (if applicable)

 

2. Complete the Online Process

Click the blue Enroll Now button below to go to our Parent Portal where you will do the following steps:

  • Set Up Your Account and Add EACH Child using the Parent Portal Enrollment Tutorial

  • Pay the Registration Fee* (Single or Family) via the "Enroll" tab   

  • Enroll EACH Child Into a Service via the "Enroll" tab (Must pay the Initial Service Fee* for one day)

Receive Registration Payment Confirmation Email
Receive Verification Email for Service Enrollment with Payment Schedule

*Payments for the Registration & the Initial Service Fees are non-refundable.

3. Submit Documents to Kids Club 360

Drop-In Space is Limited

Please note that, due to space limitations, all drop-in status children will be placed on a wait-list. Program spots will be allocated on a first come first serve basis. You will be contacted via email and phone when a drop-in spot becomes available for your child/children.

*Feeder Sites - Parents must drop off and pick up at the host Kids Club site. Transportation for feeder schools from the host site to the feeder (AM) and from the feeder to the host site (PM) are provided by D2, JICES Charter Bus, & D8 transportation and must be set up through the school by the parent.
